We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

ENROLLMENT
You are eligible to participate if you are full-time or part-time. Your coverage will be effective on date of hire.
When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical | Dental | Vision: Coverage will end the end of the month in which they turn 26
- Voluntary Life: Coverage will end on their 19th birthday or 26th birthday if they are a full-time student

- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The Benefit choices you make will remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

Blue Cross Blue Shield of North Carolina (BCBSNC) will continue to be the administrator for you medical and prescription coverage.
The plans that will be offered effective January 1, 2023 are as follows:
- HSA 3000
- HSA 1500
- PPO 1000

Preventative care services are covered 100% in-network. Preventative services included women's preventative healthcare services, well-child visits, flu shots and routine physical exams.
HEALTH SAVINGS ACCOUNT (HSA)

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
How it works:
- Pre-tax contributions are deducted through payroll and deposited into your HSA account
- You can use your HSA available funds to pay for qualified medical, dental, and vision expenses tax-free
- Unused funds remain in your account for future use & roll over from year to year
- You can change your contribution at any time throughout the year
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
HSA IRS Maximums are as follows (any employer provided HSA funds will apply to the maximum noted below):
- $3,850 for a single contract
- $7,750 for a two person or family
- $1,000 "Catch Up" contribution for those age 55+

- $1,250 for a single contract
- $2,500 for employee plus spouse
- $3,000 for employee plus child(ren) or family
Annual Employer HSA Funding-HSA 1500
- $500 for a single contract
- $1,000 for employee plus spouse
- $1,500 for employee plus child(ren) or family
Reminder: You can make changes to your HSA elections at any time for any reason. Please contact Human Resources to request a change to your HSA.

ARRIVAL provides those employees who are covered by one of the BCBSNC medical plans access to virtual visits with Teladoc. Teladoc provides members the opportunity to receive medical and mental health care from the comfort of your own home. With Teladoc, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
How much does it cost:
- General Medicine/Acute Care: PPO 1000- $10 Copay I PPO HSA 3000 and PPO HSA 1500 - $55 consultation fee

DENTAL

ARRVIAL's dental plan provide comprehensive coverage to help you and your family maintain good dental health. For 2023, the dental carrier will be UNUM. UNUM partners with Dentemax Plus/AlwaysCare to utilize their network of vision providers. Staying in-network will give you the most cost savings.

Carryover Benefit:
If an Insured submits qualifying claims for covered expenses during a benefit year and, in that benefit year, receives benefits that are less than their group’s threshold limit, the insured will be credited a carryover benefit. If an insured reaches his or her certificate year maximum benefit, Unum will pay a benefit from the insured’s carryover account up to the amount stored in the insured’s carryover account. The accrued carryover benefits stored in the carryover account may not be greater than the carryover account limit.
Carryover benefit $350, threshold limit $700, carryover account Limit $1250.
VISION

ARRIVAL offers vision coverage through UNUM to help pay for eye exams, prescription glasses and contact lenses. UNUM partners with EyeMed to utilize their network of vision providers. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

FLEXIBLE SPENDING ACCOUNT (FSA)

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care FSA
- Up to $3,050 annually
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Limited Purpose FSA
- If you open or contribute to a Health Saving Account (HSA), you may only enroll in a Limited Purpose FSA.
- If you enroll in a HDHP (High Deductible Health Plan) and elect a Health FSA, you will automatically be enrolled in the Limited Purpose FSA.
- A limited purpose FSA will reimburse you for dental and vision expenses, but you cannot claim the same expense on both the FSA and HSA Accounts.
Dependent Care FSA
- Up to $5,000 annually
- Limited to $2,500 if you are married and file separate tax returns
- Certain childcare expenses for children under 13
Carry-Over Benefit
- At the end of the 2023 year, you may rollover up to $610 into the next year
LIFE & DISABILITY

ARRIVAL provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with Unum.
Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
- Basic Life/AD&D Benefit - Employer Paid: 3x your earnings up to a maximum of $500,000
- Voluntary Life/AD&D - Employee Paid
- Employee: Choose in $10,000 increments up to the lesser of 5x your annual salary or $500,000. Guarantee Issue: $100,000
- Spouse: Choose in $5,000 increments up to the lesser of 100% of the amount you elect for yourself or $250,000. Guarantee Issue: $25,000
- Child: Choose in $2,000 increments up to $10,000. One policy covers all of your children. Guarantee Issue: $10,000
Evidence of Insurability (EOI): When you are first eligible (at hire) for Voluntary Life, you may purchase up to the Guaranteed Issue (GI) without Evidence of Insurability (EOI). Any amount elected over the GI will require EOI.
Future Increase Option: During the annual open enrollment you may be eligible fo increase your supplemental life insurance with no medical questions! If already enrolled, employees may increase up to the Guarentee Issue without EOI. Any amount elected over the GI will require EOI..
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short Term Disability-Employer Paid
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
- Benefits begin on the 8th day for an accident or the 8th day for an illness
- Benefits are payable for up to 12 weeks
- Your benefit amount is 60% of your weekly earnings to a maximum of $1,500
Long Term Disability-Employer Paid
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
- Benefits begin on the 91st day of a qualifying accident or illness
- Benefits are payable until your retirement age; if you are still working beyond your retirement age and suffer from a qualifying accident or illness, a benefit will be payable for a specified amount of time depending on your age
- Your benefit amount is 60% of your monthly earnings to a maximum of $12,500
